In my case I have run into this problem twice on my HTC Inspire 4G. Both times it was caused by adding a calendar entry from Android that was sent as an attachment via exchange email in the form of an .ics file attachment. Opening the attachment seems to do the right things on Android, but from that point forward calendar sync doesn't work correctly.
As an example, yesterday evening I added one of these calendar entries sent via email attachment and this morning noticed I was having this calendar sync issue. I then remembered what I had added last night, deleted it from my Android calendar and manually initiated another sync and things went back to normal. This probably isn't the only situation that causes this problem, but it seems to be a guaranteed way to cause it to happen twice now on my phone.
