You probably set up conditional call forwarding by typing in a string of numbers, right? I think that's your problem. Log into your GV account and next to the phone number in settings, there should be a link to disable Google Voice voicemail on that phone. Click it and it should tell you what to type into the phone to cancel call forwarding.
