Flexdialer will provide the string as entered so if there is a delay in the initial call connecting then the whole string will be off and result in the behavior you described. I personally have GV setup not to require a pin when dilaing from my cell and my settings are A,B,2,C# and it works 100% of the time.

In my brief experiments with Flexdialer Free yesterday, I first had to make sure the default number which GV uses to place a call was my Droid number, not my home number (done on the computer), although that could have been because I was using my home phone as the test number I was dialing. I also found it worked a little better with A,B,
P,2,C# as the dialing string.
