Good idea, using a CSV file
The office might have converted their PDF from an Excel or Word file ... so maybe you can get this file
I'd not do the formating in the CSV file, I'd do it in the Excel file.
For to know the right format, I'd a contact from my phone export as a CSV file and import it in Excel on the PC.
After in Excel sorted the columns in the same order as my contact, I'd export as CSV file and import it on my phone or on PC to my Google contacts.
