Do you have a photon or are you using the htc one?

If you do have the one, has this issue occurred since day one?

Are you using a screen protector? I what about a case?

Are you rooted?

What carrier are you on?

Soft reset didn't fix it?

Cleaned off the sensor recently?

Do you have a lot of home screens/live wallpaper? Low memory?

HTC one, not rooted, screen protector isn't on the sensor, sprint, nope on the soft reset, out the box 3 home screens with no live wallpaper and I have 10 gigs left of memory.
I thought that phones with proximity sensors should automatically put the screen in sleep mode when on a call so that your cheek doesn't accidentally press buttons during a call. As much as I like this phone, I'm surprised that this phone doesn't do that.
