I worked with Chase on-line support and they told me what the real error is; the quality of the picture is not acceptable. It may take multiple times of taking the pictures before it is acceptable. Make sure you are not getting any glare from your phone Flash. Dark Background, all 4 corners must be visible. I covered my flash with a peice of paper(reducing glare) and it worked well. Good Luck!
