It's always the same old apps on the Top List, even if I've already installed the app. So I hope that Android Market will give other apps a chance and remove those apps hat the user already has from the list.. that would make the market more dynamic too.
1. old download app names sometimes still remain in "My App Library" long after deletion.
2. app updates aren't immediate and automatic. i have to "check for updates".
3. 15 minute buy window.
I’ve tried AppBrain App Market... it’s somewhat better at recommending apps, but it can’t get around associating with the Market, like having to install Fast Web Installer alongside. I always end up just using the stock market, since I wind up there anyway.
The 15 minute refund window is too short in my opinion. Very often, when the program won't work properly with your phone, it takes a little more than 15 minutes to figure that out.

The 15 minute window is a pain, but a lot of devs will give you a refund of you contact them within a reasonable time and you uninstall the app.
Being a tablet owner, the fact there's no "optimized for tablets" section etc. is a bit of a pain. I used my friends iPad and the with the iTunes app, there's a special iPad section. Also, when you go to download an app that isn't the iPad version it will tell you there's a specific version available.
Device(s): Optimus-V
Rooted with Gingerbreak, Galaxy Player 5.0.

I have 2.3.6 on my phone, and the latest on my Player. I use the Market on my phone and Appbrain on my Player. If it had an option to have "My Apps" as the default screen, it would be fine. I hate the ads on the new latest Market. Once I root my Player I'll install the old 2.3.6 and be much happier.
Device(s): Galaxy S II (Sprint), Blackberry Curve 3G (T-mo), iPad 2, Chrome netbook (Cr-48), and Win 7 netbook

There are 2 things I don't like about it:
1. 15 min return window. If a game has an additional download after you install it sometimes you can't even try a game or whatever at all before the 15 min is up
2. I can't rent movies since I root my phones

I hate that I can't user Paypal to pay for apps. I also hate how you need to have your Wallet account associated with the device in order to buy from the market.
Main thing I don't like about the Android Market is that I can't organize the "My Apps" section. Specifically I'd like to either be able to delete apps (that I have no intention of installing ever again) or place them in a Graveyard folder.
Best feature is being able to surf from my PC and install direct to my phone OTA (I stopped using Appbrain once this was implemented).
Reasonable add-on I'd like to see most - batch installs. Don't need it often but if I got a new device or have to format my phone would be nice to just click off what I wanted installed from the my apps section and it would do it all in one.
Perhaps unreasonable add-on I'd like to see - being able to manage apps installed by other markets (like GetJar or Amazon). Be nice if I didn't have to remember where I got the app if I have to reinstall.
Oh yeah same feelings as everyone else about 15-min refund window.
Like everyone else said, the 15 min return policy is what I hate.
What I love, is being able to access the market with a web browser from my PC, and have it install Apps on my phone or tablet instantly just by clicking with my mouse on the web page's "Install" button.

Now I’m trying Best Apps Market, which is also better at recommending but dunno what else...
Getjar is good for looking for app deals, but not very up to date.. I'm also trying out Applorer - it has a nice UI with rather good game recommendations.
I like everything, except the fact that sometimes there are like updateable apps, but a few of them won't join the "update all" group, and you have to update them manually.

Why is that anyway?
Generally, that means the apps permission has changed. In those instances, you get to choose whether or not you update based on the new permission requirements.
