Default Exchange ActiveSync enabled by user? by specific phone? phone type/os?

Does anybody know... is Exchange ActiveSync enabled by user? Specific phone? Can it be retricted to select Phone Types or OS?

Does IT need to do anything physically on on a phone beyond entering the Exchange server after it's been enabled?
