Default Update failed AGAIN and AGAIN!! HELP PLZ

Well just like many others I am having issues with the new OTA update.
Here is what I have and what I have tried:
1.Clockwork Mod with Nandroid Backup
2. Rooted with z4
3. Metamorph
4. Rom manager
5.Root explorer
6.Titanium Backup
7. removed stock bloatware
8. de odexed
What I have tried so far...
1. Restoring to right when I just rooted my phone. NOTHING
2. restoring to a number of backups. NOTHING
3. Wipe data/ Factory reset through BootLoader (not clockwork) <--- this returned my phone to stock completely except Super User was still there. SO I tried to install Z4 and root then unroot to remove it and it worked but still no OTA.
4. Tried to download a stock image through RomManager but it would not install.

The problem I am having is the Android with the box next to him that has an arrow coming out of it pops up for a moment then the box turns to a Triangle with a ! in the middle.

Really REALLY pissing me off! It pops up every couple hours saying there in an update that needs to be installed and If I accidently hit download my phone goes down for about 5 minutes then It will be fine for a few hours then the same process.

I have searched a TON but cant find the answer.

I have RSD Lite, The SBF file, Newest (4.6 windows 7 drivers) installed but I am unable to flash a stock SBF to my phone! The phone itself does not show in the device list no matter what I do.

I'm pretty sure the newest drivers for windows are not 4.6, but 4.7.1.

OK when you open RSDLite 4.8 does you phone show up there. Did you hit Show Device?

Also you may have to uninstall those drivers before you can update to the new drivers.

When you connect the phone to the computer is your phone in PC Mode? Also on your computer go to My Computer and see if you see the Droid there. You realy need to be in PC Mode to go anywhere. Let us know.

Also your computer should search fro those new drivers on its on.

Is debugging enabled? Settings>Applications>Development>Debugging

Newest version of RSDlite is 4.8

Same thing happened to me its because of Clockworks recovery when the update tried to reboot it would just boot the phone and say it failed. I had to remove clockwork recovery then it worked fine. Just use root explorer and go to system/bin
then look for hijack and delete it then look for logwrapper and delete that then rename logwrapper.bin to just logwrapper. Reboot and update should complete then just reinstall clockwork
