Root Nand restore question

I know plenty of you have tried some of the ROMs and restored back with Nandroid restore and Titanium. What does the process entail? Does the phone go back to exactly how you left it before flashing the ROMs? What are the steps involved in this. I want to try out some ROMs and themes but don't like the thought of spending hours getting my phone setup the way it is now.
 
Exactly means apps, screens, ringtones, etc??? Where does Titanium backup come in?

If it's been awhile since you did nand, Ti backs up on a schedule so if you loaded some new apps, got updates to those apps etc, you'll get all that plus new messages etc. It's just an additional layer the way I see it. If you nand backup right before, then decide to go back right away b/c you don't care for it, then Ti is not even necessary.
