OK. I am on Grdlocks Vanilla ROM (which I love). I have noticed that available space on my SD card has decreased dramatically. I have a couple questions.

Can I complete a NANDROID backup at anytime? Or just when I flash a new ROM?

How would I use a NANDROID backup if I actually needed one?

Can I delete old ones off my SD card? I have two and they are taking up a lot of space. Do I delete the BDS folder when I delete these?

Appreciate any help.

Yes. Just boot into recovery (vol up + power)

Boot into recovery. Select Wipe. Wipe data/factory reset and dalvik-cache. Press vol down to go back to first menu. Select Backup/Restore, Nand Restore, and choose your Nand. Reboot.
Can I delete old ones off my SD card?
Yes. Navigate to sd/nandroid/HT....../
Longpress and select delete (if using rootexplorer)

Dunno.

What folders must remain on the SD card?
None as far as I know.
They most likely are holding points for your footprints (if you even use footprints), photo thumbnails, etc.
You could try backing up all these files to your computer, deleting them, and if you have problems, re-copy them back into place.
chances are they will be re created anyway as you use things like photos, footprints, web browser, etc.

I am sorry to drag this out, but I really would not like to brick my phone. Do I delete the /HT9AYHG28473 folder? It seems to me that this would delete all the backups. Or do I go 1 step further and delete individual BDS folders? Thanks for the help!

Yes you can delete those folders. I recommend keeping one nand backup, as if your phone quit working, you would have no way of connecting to your computer to transfer a backup. i keep one backup on my sd card at all times, and transfer the others over to my pc.
