Default USB device not recognized -Update

When I connect my phone via USB to my laptop or my desktop using any of the USB ports, my phone blacks out, goes to an Android start graphic, briefly shows the home view then blacks out over and over. It does charge albiet slowly. I turn the phone off so it will charge fully. In addition I get an alert from Windows "USB Device Not Recognized". Works fine when not connected via USB.

Any ideas?

Model Number ? Docomo HTC HT-03A
Firmware Version ?
1.6
Baseband Version ?
62.50S.20.17H_2.22.19.26I
Kernel Version ?
2.6.29-00479-g3c7df37 android-build@apa26 #19
Build Number ?
DRD08
Carrier ? NTT Docomo
Country you are using your phone in ? Japan

Update- Well I got the PC to connect to the phone by updating the HTC sync driver but I still can't use the phone when it is plugged in to the USB. Screen goes dark and none of the buttons function and the charge light is lit.
