I did a factory reset on my Incredible today to see if that would stop the endless reboots and am now trying to restore the screens. I had some call widgets set up that I can't seem to duplicate. I've attached a picture of the screen. Notice it says "Call <full name>" as the label and uses a full-size picture as the icon. I thought it was just a direct dial widget, but when I add one now, the picture is smaller and is overlayed by a large phone icon, and the label just has the first name.

Does anyone recognize the old widgets I had or know how to get them? I feel like an idiot that I can't remember how I did that.
