Default Damaged SD card, or card not inserted?

Every once in awhile (and again, moreso as of late) when I turn my phone on, it will say that my SD card is either damaged or not inserted. Yet, I know it's in and it's okay. I'll turn off my phone, restart it, and then it's fine.

What gives?
