Well, im using launcher pro.
icons are from here: [ICONS] Symplistic White & Black [Launcher 7][METRO UI][GO LAUNCHER] - xda-developers
And the adw minimalist theme.

clock widget is the simi widget.

power control widget is extended controls, its a paid app.

also u should get the minimal bread launcher pro widgets skin.
They look really nice.

Open your app drawer and long press an icon then drop it in the spot you want on the tool bar. You just cant change the App drawer one.
