Here are the Fields
On the Header I'd like two Entries
Event: Empty Box
Right beside it if possible on the top right put OCA #: Empty Box or choice of N/A from drop Down
Name: Empty Box
DOB: Empty Box with / that auto switches to the next entry, 03/19/2013 <-- that format
SID#: Empty Box Enough to fit 12 numbers
Address: Larger box than the others if possible, maybe 2x or 3x
Phone #: Empty Box with the Dashes. If possible have it AUTO switch to the next set of 3 or 4 numbers
Height: Empty Box
Weight: Empty Box
Hair: Blonde/Brown/Grey/White/Black/Red/Sandy/Other/Unknown
Eye: Drop Down Menu - Blue/Brown/Green/Hazel/Grey/Other/Unknown
Additional: Empty Box same as address
IF POSSIBLE is there any way to make it so i can hit a button and get another information set to pop up underneath it? For like, if I had more than 1 individual I had to write down?
