Default How have Bluetooth headset connect automatically for phone calls?

Nexus 5
4.4.2
Plantronics Voyager Pro HD

"Use for phone audio" is checked.
"Use for media audio" is checked.

But the Bluetooth headset does not connect automatically when making and receiving phone calls.

How do I make the phone connect to the Bluetooth headset automatically when making and receiving phone calls?

Thanks
