Confused Private number problem. Please help

I seem to be having a strange problem that nobody else has experienced.

My incoming calls show as private numbers, this happens to all incoming calls and I have made sure that the other numbers are NOT set to hide number.

This is my business phone so desperately need this sorted out.

I have a Galaxy S3 4.1.1 JB.

I did a software update from ICS but it did not help.

Phone is rooted

I have done some research and I have not come across anyone with this problem.

So, the caller id number says "private" instead of a number?
Does this happen with numbers you have stored as contacts also?
What rom are you on and what carrier?
How long has this been going on?
