Unfortunately that's not a single answer question. Apps go to one of two places, depending on whether they are free or paid. App data, if designed to be separate, can be anywhere on the SD card they want it. Some devs put it in one of several android system folders on the SD card. Others create their own folder.
In addition to that, the dev has to design their program to use the external SD instead of the internal one. Any apps that appear on the "On SD Card" list have the option of being moved, if not there already.
