One of the first things the phone does upon booting is a Media Scan, if you unlock quick enough and catch it you will see that occuring, it scans the internal storage then the sd card, if it fails or sees and error it will audibly alert you to that... for example, a read error occurs, the phone alerts you then retries it, if it works the second (or some subsequent time) it will clear and continue and you would miss the alarm.
The other thing you could do to verify this enable USB debugging, install the Android SDK, and run a adb logcat to see what is occuring, it might give you more insight if this continues.
