Help Galaxy S4 - Mobile Network keeps turning off

Hi all.

Android 4.4.2

Kernel 3.4.0-1529758

This is not a no signal thing. What happens is where the signal bars usually are, I get the circle with the line through it.

When I go to Settings > More Networks > Mobile Networks > Network Mode, nothing is ticked. The normal default option is LTE/WCDMA/GSM (auto connect) is not ticked either. I have to constantly check this option off. It disables itself with no discernible pattern.

This started happening a few weeks ago.

I am not sure if it was an update or what; my wife's phone so I am not sure what she may have done. Anyone ever encounter this and if yes, what is the fix ? Tried battery pull, same thing happens. Tried factory reset still happens.

How/where do I do this ?

Do I actually do a recovery and this is a step along the way ?
