Reboot into recovery ---> Nandroid ---> Backup
To restore a backup reboot into recovery ---> Nandroid ---> Restore ---> select file from list of backups

Or, better if you plan to install a rom and you use a2sd(+): nandroid backup & ext, and you will get also ext2/3 partition in a tar.gz arhive on the sdcard.

Does doing a Nandroid Backup on my HTC Desire reduce the size if the 512mb of internal memory? And can the Backup be 'directed' to my SD Card?

Nandroid is a backup OF the 512 internal memory. Including the system and data partition. It therefore would not fit on it.
