Help Notification problem in phone

Hey guys this is my first post here...

my phone shows me a notification in the "Phone" app but i double checked message, call log, etc....but i can't find any new entry anywhere. I even deleted all my messages and clear the call log but it stays the same.....I don't know what to do....plz help...!!!

Android version - 4.0.4
