If you use Dolphin HD they have a bookmark widget addon you can use. Its decent.

how did u change the last column (phone etc...) and how did you add the picture of the female? is it through root?

^^picture is through picture frame widget, and the bottom column you can drag icons to.

looks like lockbot (pro)
very cool app to make custom lock screens which you can customize completely and select different style locks (slides).

the bottom row is Launcher Pro, it's a home replacement. You don't have to root but you don't get to use the htc sense widgets while using launcher pro.

the picture of the "female" is using the stock picture frame widget....long press -> widget -> picture frame and then pick a picture from your gallery
