Help Says fully charged, but isn't when unplugged

This is an intermittent problem, which leads me to suspect my power socket might be loose or something, but I'd like some feedback anyways in case I'm wrong. My phone is no longer under guaranty but not old enough that I qualify for a replacement yet, so if I can avoid being without my phone while it goes in for costly repairs, I'd be glad for the help.

Sometimes, when I plug in my phone to charge it, and then go retrieve it when the phone tells me that the battery is fully charged (100%), when I unplug it, the battery level suddenly drops around the range it was at when I plugged it in. I'm confused why the phone tells me that it's charging and shows me a topped-off level when it's not actually doing that. In order to be sure the software isn't actually misreading the battery level once unplugged, I turned it off and plugged it in, which did show me a low battery level. Charging seems to work properly when the phone is off.

Does anyone have any idea why the software tells me it's charging when the physical battery isn't, and how to get it to give me the correct information?
 
Check if your battery looks puffed or anything. I am having the same problem except it says 100% then I take it out and it goes to 85%. Sometimes my battery increases to 90% when I havent even done anything and it turns itself off randomly and when powered back on the battery goes down to 15%. :/ My battery is puffed

I've similar issues, this being one of them at one point or another. I've concluded it's a software issue. Many other charging related issues but it seems Samsung and Android OS' do not play well together. Resolution buy a different brand. As bad as iphones are for battery issues I can;t imagine anything worse than my Galaxy Nexus. I've got two batteries and 12 chargers
