I am also stuck at 15%.
My phone boots to Download Mode. Progress stops at 15% and says my cable is disconnected. I have to pull the battery to exit Download Mode and try again. I tried a different cable and a different USB port.
If I click on the Read Phone Information button it says "Port(USB or Serial) Not Found". Is that a clue why the flash tool doesn't work for me?
During the red progress bar it says "Verifying Windows OS" and then it stops at 15%.

I don't know what else to try...

Ok I got it working. In my case I think the phone was booting to fastboot mode and not Download mode. I typed this in fastboot mode:
fastboot erase laf
fastboot flash laf laf.img
fastboot continue

(go to LG LAF Recovery tool thread to get the laf.img file)
And that got me to Download mode and then the flash tool worked perfect. Phone is retored now.

I seem to have a problem as i can go to download mode but there are text of words saying

Don't know what i did but i seem to get out of that in fastboot cli tools when i put "fastboot boot recovery.img" which boots me to recovery mode or cwm but i don't have a back up so does anyone know how to fix this error?

well nevermind got a laf.image from another website but after restoring it from the kdz file the phone is stuck at a bootloop any ideas?

Well, I tried to restore from CM11. It all went well until reboot. Now it just bootloops on boot animation. Can anyone help me on this? I'm pretty much stuck here.

EDIT:
OK, I just found out a solution by myself. Thought of sharing if anyone gets into the same trouble:

1) Turn on the phone and connect it to your PC/laptop
2) While phone bootloops, open un command prompt on pc and type, one line at the time:

adb shell
reboot recovery

This will reboot phone in recovery mode.
3) From there, delete data and cache, and reboot
4) You're done! :)

i get into download mode, get the first Flash tool screen, then the second, i hit start and it kicks me right back to the first flashtool screen not to the country select screen
so very frustrated...tried everything was so hoping i was on the right path here...
and to think prior to the last OTA i rooted with vroot/iroot in 10 min
going on about 40 or 50 hrs of trying everything

UPDATE mon 1632: rebooted winxp box (just for shiits and giggles)...low and behold flash tool is aworking!!!
success -downgraded to 10b
rooted with iroot- verified root withing iroot, rootchecker basic from playstore and (most importantly) copied files to external SD card via ES file explorer

NEVERMIND, I FIXED IT.

WHAT I DID:

If you are getting the same message when you plug in your phone into the usb "fastboot:processing commands." Dont do anything else just take out your battery while keeping the phone pluged. Then you are going to reinsert the battery BUT!! (LISTEN) you will press and hold the up button and power button before inserting the battery. THATS HOW I GOT THE DOWNLOAD MODE. Then just use the LG Flash tool and play with it. It should let you flash.

Before using this guide, please make sure you can access Download Mode by turning off your phone, then hold Volume Up and plugging in your USB cable.

You should be in download mode before starting.

Ok, lets start by download the Firmware that we're going to flash here: http://oudhitsquad.com/android/LGE/Firmware/MS323/MS32310b_00.kdz
Alternate firmware download here: Then download the LG Flash Tool here: http://d-h.st/Fph

Once you get those downloaded, extract the files from the LG flash tool zip somewhere. And to make things a little easier, put the firmware in the same directory as where you extracted the flash tool files.

Then open the LGFlashTool2014.exe up. For 'Select Type' choose 3GQCT. For 'PhoneMode' select EMERGENCY. Then go down to Select KDZ file and find the MS32310b_00.kdz file.

Now Click normal flash. A window like this should pop up:

Click start.

You should now see this:

Just click OK

A new window should pop up like this:

Your progress bar on your phone should now be moving. Congrats, we're almost done. If another smaller box pops up, just ignore it.

Simply wait for the progress bar on your phone to get to 100% and the phone should reboot, and everything will be nice and new again :D
