Help Strange Shutdown Behavior

So I noticed this just a couple days ago and it seemed to start just out of the blue. I think I was originally running one of the android police roms. Anyway I did some trouble shooting on this and it just doesnt make complete sense and Im hoping some of you might be able to shed some light on it.

***How it began***
I had my phone plugged in and sitting on my night stand like any other day. I woke up, unplugged it from power (its always turned on though) and as soon as I did the phone turned off. I then turned it back on and once it booted up it stayed on only for a few seconds and turned off. well I got to work plugged it into the power turned it on and didn't think to much about it. However I thought maybe if I upgrade the OS to something newer and better. So I installed BAMF Forever 1.0.7 then right away upgraded to the BAMF Forever 1.0.8 and then right away applied the BAMF Forever 1.0.9 patch. The hard shutdown behavior was still happening, so that seems to rule out any OS/Software issues.

On my way home from work I had it plugged into 12v power and again I unplugged from the USB end to the phone and it turned off. So I decided to do more troubleshoot. Once I had the phone back on and plugged into the 12v power I unplugged it from the lighter side and waited and waited and waited, and nothing happened. Then I unplugged the USB side of the cable and nothing. But as soon as I plugged both ends back in and then unplugged USB, it did a hard shut down. My memory is kind of fuzzy but im 99% sure with all the testing I did, that this is how it all happened.

One of the other weird things I noticed is when the phone is not plugged into power and I boot it up, for the few seconds that I can see the UI I see the battery level at "43", "45", or even "59" when its actually at a full charge. I know that because when I plug it in and boot up it will say a low number like that but in just moments it will read "100". I hope Im explaining all of this so that you all understand it, but its just so strange that its doing this. I did get to thinking what if its a faulty USB from just wear and tare? I mean its not so loose that its damaged or anything its just got a hint of a wiggle from use.

Maybe its a battery issue? But Im not sure if I buy that because its been working fine for the most part. So why all of the sudden the hard shutdowns and only booting up to the UI for a few seconds only to turn off again when not hooked into power?

Any help on this would be greatly appreciated.
 
Well even if it isn't under warranty, I do have insurance and that should cover it also. But there are a few questions I have regarding the fact that my phone is rooted.

1) Will they still fix it even though its rooted?
2) Is there a way to "undo" what I have done and bring it back to being a stock unrooted phone?
3) Or do I just have to play a dumb idioit when they ask "is this phone rooted" and go "Ummm...what?" lol

Well I got my problem resolved. Turns out it actually was the battery as I suspected. Turns out I also play a good "dumb cell phone owner". The lady that helped me noticed that the phone was rooted and I was like "whats that?" lol and played it out from there asking questions like "what does rooting do?" and such. Well as an update I not only got a replacement battery but an extended battery. This battery is usually $50 and the standard is usually $40. I got the extended at 1/2 off. :) So only $25. I drained it the first night to about 11% then stuck it on the charger over night. In the morning I turned it on and it lasted 8 1/2 hours and the screen was even on the whole time. Today I am doing it with screen on and GPS on to see how long it will last. :) Im loving the new battery.

The lady that helped me noticed that the phone was rooted and I was like "whats that?" lol and played it out from there asking questions like "what does rooting do?" and such. When you use the old battery and use any app to check battery it normally tells you bettery "health". I wonder if you ever checked to see if it said health good or some warning?
