I recall a discussion on xda about that original root file having some problems and that being one of them.

Sorry this is a root issue, he likely used the original root ROM.
Solution: Flash Root66 or one of the Verizon based root ROM, the factory reset the device. Problem solved.

I saw this problem when I rooted with invisiblek's ROM. This went away when I flashed with root66 [ROOT] PROPER Remake of Full Rooted Stock Image (aka Root66) + Easy CWM Install - xda-developers.

I believe boot will reboot your device, and you definitely don't want to use the Phone mode to flash your device, use only PDA mode or end up with a nice paperweight.

When you say gone you mean no field for it? Which should be normal for a Verizon phone.
