microSD memory card filesystem damaged

It seems like some evil force damaged file system on my microSD card.
Yesterday it was alright.
Today I have some new suspicious folders with non-sensible names like
DATA-APP.(
dpn
ppy_cross
V LEO
WIYUNz1

Some filenames have kind of DOS 8.3 format 0_0
PERFGC~1
C MMOB~1.OFF
QNDROI~1
SOCIAL~1. `
rTERIC~1
BVATAR~1.AVI

I don't remember these to appear before either...
Some folders have names consisting of impossible characters... Can't give an example because I don't have such symbols on my keyboard.
Some folders are kind of recognizable from before but got renamed in an ugly fashion:
ANDrOID
bACKUP
2gi{
astroplayerTeta
(where T have some dash on the bottom)
My Books folder also got renamed into "BOOKS $", but a new "Books" folder appeared instead and it's empty.
Well... It's not such a critical issue, but I am kind of aNNOYed...
Is it a virus? (don't think I have installed some viral stuff though)
Is it a bug?
How can I get my filesystem back in "normal" state? (please don't say "rename everything manually)
Is it safe to remove those new folders? (although it's hard to figure out which ones are completely new because android apps usually pollute sdcard root with their data and I obviously can't remember all files and folders which were in the root directory of my microSD card before)
How do I prevent this from happening in future?
---
My phone runs Android 4.0.3, no custom firmwares..
I haven't removed my SD card from my phone ever since I inserted it quite a while ago

Do you remember the name of the app you installed?
All apps have permissions to do certain things, one of which is the ability to modify sd card contents.
See HERE for app permissions list and what they do.

My best guess is that the app you downloaded had the ability (permission) to modify your sd card contents and unless someone knows of a different answer, I would suggest a factory reset.

Yes you will lose your apps and contacts (unless backed up to your google account), but at this point, I see no other way.

And you do not know the name of your device?

omg I've mentioned it in my previous message, it is Alps S4001
I know about permissions, but almost every application I ever installed requires lots of them. And almost every application requires sd card permission. They all do. Readers, backup utilities, music players, video players, voice recorders, system monitors... they all do, even those which theoretically could work without SD card at all
BTW on Windows all applications have permission to modify whatever. And I have no filename issues on my PC; neither had I on Windows Mobile (which also does not care about permissions) phone

How long have you had the SD card? It could be a bad card...
It has been about 18 months since I bought it; capacity: 32 GB

P.S. I've just thought: Even if I reset my phone, I will have to install my music players and readers again... and grant them permissions again
