I have my phone on B09 Stock ROM right now, with stock recovery, but it's rooted.
I got the OTA update alert and downloaded it and tried to install it. I got an error:
"Finding update package...
Opening update package...
Installing update...
Verifying current systen...
assert failed: apply_patch_check("EMMC: .....
... (Status 7)
Installation aborted.
E: Failed to open /cache/fota/error_code (No such file or directory)
E: Failed to open /cache/fota/source_code (No such file or directory)

Could someone Help a guy out?

I was able to flash using CWM and was running 4.1 off of that for a day. I had some issues with Qualcomm and Wipe and was able to freeze them with Rom Toolbox. I am not sure why, but after that I was experiencing lag issues. I was completely stock aside from rom toolbox.

I decided to go back stock B09, since I was able to maintain root, to check it the OTA was available yet. It says that there is no update available. I used the ZTE Update tool and got a message saying that my device is already running B11.

Does anyone know a way to change this back to B09 and get the ZTE Update tool to read my phone as only being on B09?

Thank you in advance. I am also sorry if this has already been covered somewhere else. I skimmed through the couple threads regarding 4.1 and did not see anything.

If you flashed to OTA 4.1.2 then you also flashed the new partitions that were included outside of the normal system patches. If you then flashed back to B09 then you would still have the new 4.1.2 partition updates in place which is probably whats throwing the updater off.

Follow this thread Here
particularly on page 2 i believe theres a post or 2 about returning to the stock B09 partitions

I appreciate it. I was able to find the stock partition on page 3. It worked just fine. I was able to return completely to stock and am just running the update from ZTE.

Thank you. I appreciate your assistance. I was able to find it on page 3. The stock partition. I am going to go ahead and update from ZTE because it is B14. I am not sure what that means yet. Still downloading. But, the file name is N861V1.0.0B14.zip
