Root [MetroPCS/CDMA] sd card keeps malfunctioning

Have you tried a different SD card.I think Lexar might be your problem.

I had the same problem with a 16 GB SanDisk.. I brought it back to Target (where I purchased it) and exchanged it for another one. The replacement worked prefectly, and the problem was definitely a faulty card the first time around.
