2nd lock screen issue

Okay... So looking back at your original screenshot I can see there is an app with a notification at the top left of your status bar. Those icons come from user apps.

If the culprit app requires having a persistent notification to run a lockscreen, then that might be the app in question.

Can you pull down the notification shade and see what app is putting that icon in your status bar?

And one more thing. I was curious about CCleaner so I just looked it up on the play store. Not only does it run on startup, it has permission to display on top of other apps.

This really might be your problem app.

Regardless, even if it isn't, I really suggest getting rid of that app. The permissions it needs are pretty sketchy in my opinion. And really, you don't need these types of apps on Android and it most likely causes you to get worse battery life and not better.

The issue appears to be fixed. I'm not really sure what happened but I did restart my phone and have not seen the slide to unlock screen come on in about the past hour. I will say I did delete the CCleaner app but must protest that the issue started well before that app was installed I tried to use that to fix the situation but ultimately must admit that the screen issue went away after I uninstalled it and restarted the phone. Thank you for all your help!!
