Attempt to mount it using the phone settings and tell us exactly what happens (i.e. any messages).
Otherwise: Reboot. Remove the card and re-insert it. Check whether a different device can read it, or whether it can read a different card (i.e. is the problem the card, the phone, or the combination of the two?).
