I upgraded to Froyo following the instructions from the following link:
Download And Install Official Android 2.2 Froyo On Samsung Captivate

After the upgrade, the add contact form is gone back to the 90s. There are only 3 fields, Name, Phone and E-mail. The name field is too limited for long names. Phone field has no options for different types of phone numbers that one may have (Mobile, Home, Office etc).
Surprisingly, editing my existing contacts still shows the old form. The problem seems to be with adding new contacts only.

Just figured it out! For some reason, the phone was trying to save new contacts on the SIM. Changed the setting from "Contacts >> Menu >> More >> Settings >> Save new contacts to >> Phone" and add contact form is back to it's self again.

That's a rather old leaked beta. Many people have had issues with it. If you're going to go the route of flashing an unofficial ROM to your Captivate, you're better-off using one of the current hacked custom ROMs. They use much newer code and have numerous improvements.
