    I tried

    Name: Consumer cellular
    apn: att.mvno
    Proxy: proxy.mvno.ccmobileweb.com
    Port: 80
    user name: not set
    paswd: not set:
    server: http://mmsc.cingular.com
    MMSC: http://mmsc.cingular.com
    MMS Proxy: 66.209.11.32
    MMS Port: Not set
    MCC: 310
    MNC: 410
    Authentication Type:pAP or CHAP
    APN Type: Not set

which works on my S3 but not on my S4. Anybody get Consumer Cellular to work? (Voice mail and texting isn't working).

My parents have Nexus 4s on Consumer Cellular and I happen to be visiting tomorrow. I'll take a look at them tomorrow and post what they have then.

APN settings are normally carrier-dependent, not device-dependent. (Are you certain you don't need an APN Type? I've never seen that blank before.)
