Apps Creating a map flag/bubble like in the maps app

I need to create a flag for the pins on my map similar to the flags used in the google maps app (or more precisely like the ones used in the maps app on the iphone). What is the best way to do this?

I having been messing around with the ItemizedOverlay for the past couple hours but the documentation for how to use it (and even the explanation for what it is for) is very poor. Is an ItemizedOverlay what I should be using? Should I just use a second Overlay? Or is there another option I haven't thought of?
