Data management app that came with the phone?

I've managed to lose the data management app that came with the phone....no clue what happened or where it went to. It was a simple button that had a red bar at the top and a white box below it that showed data usage for on air (3G and I'm not sure if it tracked 4G also, but it didn't count Wifi, only air usage). It was the one that came with the phone, so I'd like to use that one rather than installing and relying on another app. Any idea on how to get this app back without having to restore the entire phone?

Thanks in advance for any help!

EDIT: Sorry, running Android version 2.3.5 on a Verizon Spectrum

Was it a widget on one of your home screens? If so, then long press on an empty space on your home screen and select widgets and see if it is listed. Check your app drawer and see if it is in there.
