I am installing a lightweight PDF reader, and when I try to access a local PDF file, it asks for "All File Access" from Android. Does reading a single file type require an app to have All Files access?

Yes. There isn't a specific permission for each possible file type, and pdf doesn't count as "media", so you need this permission.
Just speculating, but a lot of 'PDF reader' apps have the ability to view a lot of other similar file types (epub, azw3, odt, etc.) so perhaps that 'All Files Access' is just a generic reference to allow the app to open up other files that are in that same general 'text and postscript' category.
