I'm thinking this is way over my head, but I'm gonna take a stab at it. I have an app (or widget?) on my Samsung galaxy S4 called "FindMyPhoneProvider" 1.03. What is it? Can't find any reference to it on the web and don't know how it got there.
I don't see anything like that on my Nexus 4 so personally, I'd be tempted to uninstall it - if that's possible, failing that, disable it. .. but maybe S4 owners will have another view
I have a Verizon Branded S4 and do not have that app on my phone. It could be something that is carrier specific or it could be one of those apps that sneaks in on the shirt tails of another app. A quick call to your carrier will verify(or not) the first option. If they aren't responsible, I would certainly uninstall it immediately.
There's a seemingly defunct website with that name, with very little information about what it is... http://findmyphoneprovider.android.informer.com/
