Help Headphone Icon - Galaxy S3

My headphone icon keeps popping up on my new galaxy S3. Sometimes the music player plays by itself ... what can I do to remove the headphone icon? and How do I get the music player not auto play? I have seen the settings but it shouldn't start playing the music on its own.

It appears as if the "headphone inserted" sensor is triggering.

I moved this to the Samsung Galaxy S3 section where it will get more eyes.

Try inserting a headset into your headphone jack and then remove it. If there is an intermittent connection, that might clear it up. You also might be in range of a Bluetooth device that is active that the phone thinks is a Bluetooth headset. The easiest way to troubleshoot that is turn off Bluetooth and see if that fixes your issue.
