Unfortunately, the HTC Mail client for the Desire doesn't support IMAP Idle (instant notification of mail updates) - can anyone recommend an alternative mail client?

I will second that. K-9 is a brilliant app, developed by a responsive team and is free. It handles multiple IMAP accounts and the push mail is very effective with a minimal drain on the battery. If you don't want to be bothered in the night, you can also get a plug-in for Locale / Tasker which will allow you to control push and poll email times.

Have installed K9 but after entering an email I can't see how to actually send it? There doesn't appear to be a send button.

Yep, found it - not very intuitive!

A few things about K9 are not that intuitive but it's better than anything else out there at the moment. It's more of a 'pet' project (pun intended) by a few people that concentrates more on functionality than looks/interface.
