Here is the behavior I am noticing:

    1) The time on the lock screen doesn't update, so if the phone powered up at 11:08am that is what the time is stuck at no matter what.

    2) The lock screen shows "(No service)" but the phone can answer calls.

    3) The lock screen is unresponsive - trying to unlock it, or move the other slider (to change vibrate/sound) does nothing.

    4) The apps in the background seem to be running and seem to be running.

    5) If I press the power button the screen turns off, but will turn turn on again when I press the power button another time.

The phone has the stock ROM, but it is rooted.

Has anyone else ran dealt with this issue?

No. Are you using a lock screen app? If not, wipe cache, delvic cache in recovery and reboot.

No lock screen app. I ended up just using R3L3AS3DRoot to just start all over. I have been messing around with the phone so I figure a "clean" start is good.

Thanks for the suggestion, if someone else has this same issue maybe they can try the ideas you suggested and report back if they work.
