Apps Location app android , choose a place and add title/description for that place

Hi dear,
could someone give me a hand-help , I'm really need this code source template

I want to build an app that have a screen that have two buttons: 1st button = open map, select place , add title and description of that place, all data saved in Mysql database.

2nd button = show all the places in map that we have in our database or added by all the users.

this code will be done in php and Android as u know

please if someone know how to do that . please don't hesitate to give it to me .
