I installed a new micro sd card on my Samsung Galaxy S2 and then formatted it on the phone. I'm having issues when connecting my SGS2 to my pc via usb:

I go to applications-settings-wireless network-usb utilities - click on connect storage to pc, and then connect it to my pc via usb. And then I get the following message notification on my SGS2 phone "memory card removed". I have no idea why this is occurring and how to fix this?

That happens because the in USB mode your mounting the memory card as a drive to your computer. For that to happen it has to unmount from the phone. Pretty much your turning your phone into a memory card reader.

what OS are you running on your pc? I know when running XP and Vista i have issues with the mounting part, but with Win 7 it loads as a Multi storage device. To over come this with Xp/ Vista you can down load the drivers from Samsung...
