Somebody takes a photo using their phone - and does nothing else.

At some later time, on another device, they open their facebook page and click Upload Photos.

Somehow, that photo they took using their phone is available for upload.

    The Question

Where is that copy of the photo and how did it get there?

They have automatic syncing of photos to some cloud service active (Dropbox, G+, or many others).

The Facebook app has a 'sync photos' feature in its settings. If that's active, the contents of your phone gallery is synced to a private album ready to share whenever you want. The private album is on Facebook servers so its available on any device you have opened your Facebook account on. You can turn the feature off in the app (I have it off).

Advice would be, next time an app updates, and you opened it, read the new prompt. Facebook asked me if I wanted to turn on sync photos from the get go.

Apart from Facebook, others also offer the same service, as Hadron said.
