Here is mine. Nothing all that exciting. The wallpaper was put together from sources by Nick TexasLTE. The icon set is by SeriouslyCrazy on XDA, packaged up nicely on the Play Store HERE. The clock is just Beautiful Widgets. Nova Launcher on Dirty Unicorns, of course.
Keeping it boring and simple right now because I'm happy with my phone for once.

Icon: Tiny White
Other: PopUp widget for the music and weather at the top, zooper for the darker area on the bottom, Paranoid Android#s Per App colour for the white status bar.

Do you have to be rooted to do all this?
Only for certain elements.
Most things are accomplished with just a third party launcher, and/or things like zooper or uccw widgets, and dont require root.
Things like changing your softkeys, or on some devices hiding or making transparent the menu bar/dock requires root.
Here is mine
Still using the stock GE launcher. Using an app called desktop visualizer. Basically you can change icons without having to use a different launcher like nova or Go launcher for example.
