Recently rooted HTC Desire (CDMA). Still running the stock rom, just wanted to remove some of the bloatware on my phone. I installed Titanium Backup, ran a batch to backup all apps+system data. I uninstalled City ID, rebooted, and just for fun I went back into TB to see if I could restore it. To my surprise it wouldn't let me. Next I tried uninstalling the reddit app. Rebooted and it allowed me to restore that one. Does TB not restore apps that came pre-loaded on the phone?

System/app is the location of pre loaded apps. The back.up is "all user apps"

    User apps are in data/app

You could copy the .apk to somewhere safe or even move it to data/app.
