My phone is refusing to be recognized by my computer (I get the "USB devise malfunctioned" notice), which, of course, makes it impossible to use Odin. I need to return my phone to stock in order to sell it. I tried flashing the "FA19.zip" file in CWM, but got a MD5 mismatch error. Any ideas???

Try switching usb ports, usb cables, different computer etc. Sometimes a connector gets a little screwed up.

Where are you getting this FA19.zip from? As far as I know, you can't erase clockworkmod through clockworkmod. I think the best you could do is flash halfassrom onto it, its fairly stock. Besides, if you are selling a phone with a broken usb port, it probably won't make a difference if its modded or stock, its still broken.
