Looks like a graphic rendering issue. Do you have graphics type issues in other areas? Like games and such? Or does it only happen on the homesceen?

If it doesn't, it could just be the TouchWiz launcher. Try downloading a Nova Launcher (free version). See if the problem still persists. If it disappears, it's the Touchwiz launcher. You can try to do a system cache wipe to see if it would fix it:

Turn phone off, then press volume up and home while powering up. Do not let go of the buttons until you see the spinning Android blue screen. You may be presented with a dead Android screen for a minute but don't mind it. Then you will see the recovery menu. Use the volume buttons to navigate and select 'cache wipe'. Be careful in this as it's right next to 'factory wipe'. Then once highlighted, press the power button to select, and confirm the action. Once done, select power off the phone, then turn it on again.

    Hi, thanks for the tips.
Wiping the cache does not help. Same issue with Nova Launcher...

It does happend inside apps/games also. If i do a factory wipe the device will act normal for a few days.. and then the issue return.

This is a picture from the play store for example; http://imgur.com/a/Kduz6

    Thoughts?

If it happens everywhere, there is no guarantee that a factory wipe can fix it. There is that probability but I'm leaning more toward a hardware problem with the graphics chip. Since sending it in for repair would involve you doing a wipe anyway, try it, and if doesn't help, send it in to the shop.
