Ok, so I have tried searching the forum for an answer, but haven't found one.....
When my wife makes a call and proceeds to do stuff on her phone, the other caller can hear every time she presses a button, all of the music of any app she is in, etc.

    She has a 16GB Galaxy S4 from T-Mobile

My 16GB Galaxy S4 from AT&T does not do this, so it doesn't appear to be a phone issue, all that I can think of is that there is some setting somewhere.

I have already checked the receive alarm and message notifications while on call and that is turned on for both phones, so I don't think that is an issue.

    Anyone have any ideas?

Perhaps your Noise Reduction has been activated. Check this detail article on all the "call" feature on your S4 device:
How To Use On Call Options On Samsung Galaxy S4 - P^i

    1.Go to Setting
    2.Go to My Device tab
    3.Open “Call”
4.Under the “Ringtone and sound settings”, enable “Noise Reduction”
5.It super passes background noise from your side during calls

No, that is already checked. It is really strange, but the other person can hear all of the beeps, clicks, etc. when she does other things on the phone.
